Care Manager of Health Home Care Management

$23 - $26.37 per hour

Care Manager Job Description Sun River Health, a Federally Qualified Non‑Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester counties, is seeking a full‑time Care Manager to join our team in Peekskill, NY. Key Responsibilities Work closely with the interdisciplinary care team—including PCPs, mental health providers, residential services, substance abuse providers, and others—to develop and coordinate care plans. Collaborate with the Patient Navigator to direct field activities and ensure optimal flow of information across the care team. Provide input to providers, patients, and families for written individualized care plans. Review patient intake assessments and use results to coordinate completion of care plans, self‑management goals, and strategies. Identify potential barriers to care with patients and help them overcome these barriers; follow up with patients who have not met treatment goals to resolve issues or adjust goals. Evaluate medication compliance, assess adherence barriers, and ensure medication reconciliation is current. Receive alerts for inpatient and ER admissions, visit patients during inpatient stays, and actively participate in discharge planning and care transition activities. Contact patients within one business day after discharge from inpatient services or ER. Help patients keep scheduled appointments and arrange appropriate metabolic and preventive screenings per agency policy. Ensure patients and caregivers are aware of test results by facilitating discussions between patient and physician as needed. Coordinate services between patients and extended care team providers to ensure the integrated care plan is fully implemented. Regularly review workload reports in TREAT to identify patients requiring assessments, outreach, and engagement. Provide or arrange self‑management/wellness education, peer and other support groups in the language preferred by the patient or family. Organize and participate in case conferences as needed and in accordance with agency policy. Review benefits, entitlements, and housing with patients or families and assist in the application process, following up as necessary to ensure services are approved. Utilize the TREAT system to complete all documentation and assessments timely, including scheduling of all activities. Qualifications and Experience Minimum Education: High School diploma or GED. Preferred Education: Bachelor’s degree in Health or Human Services related field. Minimum Work‑Related Experience: 2 years in a similar care coordination role. Compensation and Employment Full‑time position. Salary: $23.00 – $26.37 per hour.
