The OOCN collaborates with physicians, families, and multidisciplinary teams to develop holistic care plans. Respond promptly to provider communications, document interactions, and escalate concerns. Deliver emergency response and follow-up support. Contribute to quality improvement through feedback and staff training participation. Ensure end-of-shift communication with the Clinical Director to maintain continuity and prepare for morning meetings. The night nurse will be responsible for managing DME and making sure supplies are delivered in a scheduled, timely manner. The OOCN will be on-call Sunday through Thursday from 4:30 pm to 8:00 am.Essential Job Functions:Nursing CareAdminister wound care, IV Hydration, lab draws, and other nursing tasks during home visits, ensuring the best outcomes for participantsWe in the service area when on shift ready to answer any phone calls and make any visit.Coordinating CareCollaborate with physicians, family members, and multidisciplinary teams to create a holistic care plan that addresses all aspects of the participant's healthResponding to CommunicationAnswer calls from primary care providers and other staff promptly, triggering concerns as needed and documenting all interactionsEmergency Response and Follow-up CareTake immediate action in emergency situations and provide ongoing support for participants.Participant in Quality ImprovementContribute to continuous improvement efforts by providing feedback and participating in staff development and training initiatives.Communication with Clinical DirectorCommunicate with Clinical Director before the end of shift to ensure clear communication for morning meetingQualificationsQualifications and Experience:Must be at least 18 years of ageMust be a graduate of an accredited School of Nursing and currently licensed to practice in KansasMust have a valid Kansas driver's license and current car insuranceMust have strong clinical, communication, and critical thinking skillsMust be proficient with Microsoft Word and OutlookMust have current CPR certification, or be willing to obtain it within 3 months of hire dateMust have the ability to work independently, manage stressful situationsMust be able to provide excellent customer service to participants and familiesPrefer at least 2 years of home care nursing experience, including with the frail elderly populationPhysical Requirements:Medium Work – Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently and/or greater than negligible up to 10 pounds of force constantly.
