Overview Key Job Duties and Responsibilities The Licensed Practical Nurse is to provide direct nursing care to the residents in accordance with current federal, state, and local standards, guidelines, and regulations that govern the facility, and as required by the Director of Nursing Services or Nurse Supervisor to ensure the highest quality of care. The following is a general list of key job duties and responsibilities. This list is not inclusive and other duties may be assigned by an authorized supervisor. Responsibilities Comply with the Facility’s personnel, safety, and corporate policies and procedures. Comply with the nursing department’s policies and procedures. Report witnessed or reported allegations of resident abuse/neglect immediately to the Administrator, Director of Nursing or Unit Manager. Treat residents, family members, visitors, and team members with dignity and respect. Maintain the confidentiality of all resident and family information. Report to work on time and as scheduled. Assist in the admission and discharge of residents. Assist residents in daily hygiene care including bathing (bed bath, shower, tub), oral, ear, eye, facial, hair and nail care. Keep resident’s dry by changing clothing and linen when soiled or wet. Assist residents in bowel and bladder functions; collect specimens when required. Assist in transporting residents to/from appointments, activities, and social programs. Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc. Report and document any changes observed in a resident’s condition or behavior to the Charge Nurse; assist in the care of dying residents and provide post-mortem care where applicable. Administer professional services such as catheterization, tube feedings, suction, applying and changing dressings/bandages, colostomy and drainage bags, taking blood, etc., as required. Perform lab tests, blood pressures, and other routine monitoring as required per the resident care plan; admit, transfer, and discharge residents as required. Complete accident/incident reports as necessary. Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, and related paperwork; chart nurses’ notes in an informative and descriptive manner reflecting the care provided and the resident’s response to care. Fill out and complete accident/incident reports; submit to Director as required and follow established procedures; report discrepancies concerning physician’s orders, diet changes, or charting errors to the Nurse Supervisor; perform routine charting in accordance with policies and procedures. Develop and maintain good working rapport with inter-departmental personnel to ensure nursing services meet residents’ needs; complete required documentation of care and services as provided. Review complaints and grievances filed by assigned personnel and make appropriate reports to the Nurse Supervisor as required. Receive/give nursing reports at shift change and report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals per policies and procedures. Monitor assigned personnel to ensure adherence to safety regulations in the use of equipment and supplies; recommend equipment and supply needs to the Nurse Supervisor; review care plans daily to ensure appropriate care is rendered. Maintain confidentiality of all resident care information; oversee resident needs, staff, and operations as needed. Essential Job Duties / Physical Requirements The following is a general list of key job/physical requirements for this position. Other requirements may be necessary as deemed by the Facility. Regular worksite attendance. Must have a valid professional license (LPN) in good standing with the State. Must have a valid CPR certification. Must have basic computer skills; must be able to read, write, speak, and understand English. Must be able to work beyond normal hours and on weekends/holidays when necessary. Must be able to assist in the evacuation of residents during emergencies. Must be able to perform the essential position functions with or without reasonable accommodation. Employee may use a transfer/gait belt or require assistance when lifting or carrying objects over 25 pounds; some body movements may occur naturally in routine duties. Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and sometimes hostile people; may involve working within the community setting. Must be able to see and hear, or use appropriate prosthetics, to perform the job effectively.
