Adult Medicine Physician

Join a physician-led,value-based primary care organization dedicated to delivering high-quality,patient-centered care to adults and seniors in Green Valley, AZ. We are seeking a Board Eligible orBoard Certified Family Medicine or Internal Medicine Physician to become partof a collaborative care team focused on improving health outcomes throughpreventive care, chronic disease management, and coordinated treatment.
Practice alongsideexperienced Nurse Practitioners, Care Managers, Pharmacists, Social Workers,and Clinical Support Teams in a supportive environment designed to helpphysicians focus on delivering exceptional patient care.

Key Responsibilities:
- Deliver comprehensiveprimary care services for adult patients.
- Diagnose and manage acute,chronic, and complex medical conditions.
- Interpret diagnostic testsand coordinate specialty referrals when appropriate.
- Collaborate withinterdisciplinary care teams to optimize patient outcomes.
- Promote preventive care,wellness, and chronic disease management.
- Participate in qualityimprovement initiatives and patient engagement programs.
- Maintain accurate and timelyelectronic medical record documentation.
Qualifications:
- MD or DO from an accreditedmedical school
- Active Arizona medicallicense (or eligibility to obtain)
- Current DEA registration
- Board Certified or BoardEligible in Family Medicine or Internal Medicine
- Passion for deliveringvalue-based, patient-centered care
- Excellent communication andcollaborative care skills
