The position requires providing education to patients, families, and healthcare team members, while ensuring compliance with professional standards and organizational values. Job Details Certifications, Education Requirements, Degree Requirements, Location Requirements, and Years of Experience: Degree in Physical Therapy from an Accredited School; 1+ Year of Outpatient Clinical Experience; CPR Certification. Schedule: Days shift, 8 hours per day, minimum of 40 hours per week. Completes comprehensive evaluations of assigned patients and develops individualized treatment plans focused on progressing patients towards discharge within an established length of stay. Performs accurate and timely documentation, including goal writing, functional assessments, and medical necessity letters, in compliance with all applicable guidelines and policies. Ensures care is provided under signed, valid physician orders and within insurance approval parameters. Participates in student educational programs, creating positive learning environments with appropriate supervision and evaluation. Actively pursues professional development through continuing education, self-study, and involvement in professional organizations. Adheres to all relevant State Practice Acts and maintains professional licensure in good standing, completing continuing education requirements as mandated. Demonstrates understanding of the organizations performance improvement initiatives and contributes feedback to enhance service quality.
