Overview

Avento Health is recruiting a Manager of Case Management for an established hospital in Houston, Texas. This leadership position supports an adult hospital-based case management program encompassing care facilitation, utilization management, case management, and discharge planning.

The Manager will coordinate daily operations, develop case management professionals, promote effective use of hospital resources, and help improve clinical, financial, utilization, and quality outcomes.

Responsibilities

• Coordinate daily hospital case management operations and workflows
• Support implementation and management of the local case management program
• Oversee care facilitation, utilization management, case management, and discharge-planning activities
• Promote appropriate and effective use of hospital resources
• Use financial, utilization, and quality data to guide operational decisions
• Develop and implement performance-improvement strategies
• Support hiring, performance reviews, staff development, and personnel functions
• Lead and coach an interdisciplinary team of nursing and social work professionals
• Facilitate collaboration among physicians, hospitalists, nurses, care managers, and community resources
• Help achieve targeted clinical, quality, utilization, and financial outcomes
• Maintain compliance with departmental policies, service standards, and budget expectations
• Serve as a mentor, preceptor, and resource to less-experienced team members

Required Qualifications

• Bachelor of Science in Nursing with a current Texas Registered Nurse license, or social work education qualifying the candidate for a current Texas Licensed Master Social Worker license
• Current Case Manager Certification
• At least five years of experience in utilization management, case management, discharge planning, or a related cost and quality management program
• At least three years of hospital-based nursing or social work experience
• At least three years of demonstrated leadership experience
• Knowledge of payer requirements and leading clinical care practices
• Working knowledge of managed care, inpatient care, outpatient care, home health, utilization management, and case management
• Strong written, verbal, leadership, and interdisciplinary communication skills
• Ability to build effective working relationships with physicians and healthcare professionals at all levels

Preferred Qualifications

• Master’s degree
• Licensed Clinical Social Worker credential
• Experience leading case management operations in a fast-paced adult hospital environment
• Experience with performance improvement, business planning, and targeted outcomes
