We are seeking a Licensed Vocational Nurse (LVN) to join our team in Santa Clarita for evening or overnight shifts. The LVN will provide essential patient care in home health care settings, ensuring accurate monitoring and documentation of patient progress. This role requires occasional local travel to support patient needs effectively.
Responsibilities
- Monitor patient vital signs and conditions regularly
- Maintain accurate and detailed charting and documentation
- Assist with patient care activities in home health care settings
- Communicate effectively with patients and healthcare team members
- Adhere to established nursing protocols and standards
Preferred Qualifications
- 1+ years of experience as a licensed vocational nurse
- Valid LVN License
