Job Responsibilities:

? Provide direct care and behavioral support to clients

? Assist with daily living activities including grooming, meals, and toileting

? Monitor patient safety through routine observation and rounding

? Assist with ambulation and transfers as needed

? Monitor vital signs and report changes to medical staff

? Provide crisis intervention and behavioral support services

? Implement individualized behavioral plans

? Encourage client independence and participation in treatment programs

? Maintain accurate documentation per facility policies and procedures

? Collaborate with nurses and other healthcare professionals to support patient care

Qualifications:

? High School Diploma or GED required

? Minimum 1 year of Behavioral Health, Mental Health, DSP, CNA, or related experience preferred

? CPR certification preferred (if applicable)

? Strong communication and interpersonal skills

? Computer proficiency required

? Understanding of HIPAA and patient confidentiality requirements

? Must be at least 18 years old

? Must meet all federal, state, and local requirements
