Hospice Nurse

Location: San Francisco County, San Mateo County, and Santa Clara County

Assignment Duration: This is a 13-week contract position.

Schedule: Days: Sunday - Thursday Hours: 10am - 7pm or 11am - 8pm (Schedule can be flexible based on patient needs)

Job Description: The Hospice Nurse is responsible for admitting patients to service and assessing the needs of the patient and family, including physical, psychosocial, environmental, safety, and developmental needs. They will develop an individualized plan of care based on their assessment and will document observations and interventions related to the patient's environment. This role requires coordination of care with a multidisciplinary team, participation in the hospice program's quality assessment performance improvement program, and conducting follow-up visits as needed.

Responsibilities:

• Admit patients to hospice services.
• Assess patients' and families' needs comprehensively.
• Develop and document individualized care plans.
• Coordinate care with a multidisciplinary team.
• Participate in quality assessment performance improvement programs.
• Conduct follow-up visits as required.

Required Experience: One year of hospice experience is required.
