Summary of position

This individual will make a difference in the lives of many by assisting people with intellectual and developmental disabilities to access and coordinate needed support and services in community settings. Knowledge of social, economic, health or habilitative services is important, as is knowledge of methods of compiling, organizing, and analyzing data. This person must have strong problem-solving skills and the ability to plan and develop individualized service plans. The support coordinator must evaluate and monitor service delivery and implement corrective action plans and must possess the ability to provide technical assistance regarding program and client issues. An effective communicator, this person will be able to establish and maintain effective working relationships with others and to understand and apply applicable rules, regulations, policies, and procedures.

Essential Functions

• Assists people with intellectual challenges or developmental disabilities to access and coordinate needed supports and services in community settings.

• Coordinates and facilitates all activities related to gathering information to be included in the ISP using the Person-Centered Planning Tool (PCPT)

• Arranges and obtains clinically pertinent assessments.

• Reviews clinical information and recommendations for interventions and services prior to the ISP meeting.

• In collaboration with members of the clinical intake and assessment team, assists the individual and family in developing and implementing the ISP within 30 days of enrollment in the Waiver.

• Scheduling and facilitating planning team meetings to develop the ISP, distributing the ISP (and PCPT when the individual consents) to the individual, all team members and identified service providers.

• Identifies the individual’s vision of the future; functional/health status; identify preferences; capacities; interests; needs; and personal goals; relevant services; supports and activities in the ISP and the PCPT.

• Reviews the NJCAT (New Jersey Comprehensive Assessment Tool) with the team for accuracy.

• Accesses community and internal agency resources (clinical professionals, etc.) in the process to identify and attain needed services.

• Assist families to access generic services prior to the Division funded services.

• Provides monitoring and reviews of the ISP and PCPT though monthly monitoring conducted at specific intervals.

• Obtaining authorization from SC supervisor for Division funded services.

• Ensures at least an annual review/update that summarizes the prior year’s plan.

• Assists the person, family, or advocate to identify and access the most cost-effective services and community supports based on the individual’s and family’s interests and preferences. These services must not be a duplication of services that can be provided through local, state, and other federal agencies. Services will be utilized in accordance with the parameters defined in the Supports Program and the Community Care Program Manuals.

• Assists individuals and families in applying for/maintaining Medicaid Eligibility.

• Complete the Medicaid Troubleshooting form as needed.

• Monitors annual Medicaid redetermination dates and follows-up with family/guardian regarding the completion of Redetermination process. Refer the individual/family to DDD- Medicaid Helpdesk when necessary.

• Maintains necessary documentation regarding eligibility in consumers’ records.

• Accesses community and internal agency resources (clinical professionals, etc.) in the process to identify and attain needed services.

• Oversees the person’s participation in the services, support and activities delineated within the ISP.

• Determines the extent to which supports, and services meet the needs and expectations of the individual and family utilizing the required monthly monitoring report.

• Meets face to face with the Individual quarterly. One of these visits must be at the residence. One visit to the Day program must be completed if the individual attends a program on a regular basis.

• Documents all contacts/services in the Monthly Monitoring tool and in I Record Notes in a timely manner.

• Ensures a complete, up-to-date ISP and PCPT

• Ensures all services requested tie into the ISP and the PCPT.

• Intervenes in a timely manner when the individual, family or advocate identifies a problem and reports the issue to the Division.

• Submits all required documentation in accordance with mandated deadlines.

• Conducts business in a professional manner in accordance with internal policies.

• Maintains compliance with Monthly Monitoring deadline with uploading contacts within 24 to 48 hours post contact and following the weekly percentage contact and upload requirements with a total of 100% before the end of each month.

• Reviews caseload monthly and completes service renewals 30 days prior to the due date.

• Always maintains the confidentiality and integrity of the company, complying with HIPPA, DHS-DDD.

Position Requirements

1. A bachelor’s degree or higher in any field.

1. A minimum of 1 year of experience working with adult (18 or older) individuals with intellectual/developmental disabilities is required. This experience must be the equivalent of a year of full-time documented experience. It may include volunteer experience as well as being a family caregiver.

1. Mobile Worker: This is a mobile position, meaning that regular travel to different work locations is essential. Will be exposed to weather conditions typical of the location and may be required to stand and/or sit for long periods of time.

1. Reside in the same territory they are assigned to work in; exceptions may be considered, due to business need.

1. Occasional evening and weekend work may be required as job duties demand.

• Ability to use Microsoft Office 365 applications.

• Knowledge of planning principles and report preparation.

• Skill in interpreting, policies, and procedures.

• Skill in analyzing situations, evaluating information and recommending course of action.

• Skill in maintaining effective working relationships with fellow employees and outside agencies

• Ability to evaluate issues, explain rules and regulations.

• Ability to write reports and correspondence.

• Ability to effectively present information and respond to questions from groups of managers and associates

• Ability to define problems and collect data, establish facts and draw valid conclusions.

Travel/Work Environment

1. Remote position with required travel throughout Essex, Hudson and Union counties.
2. Must reside in one of the assigned counties.
