Nurse Practitioner/Physician Assistant - Primary Care (Full-time or Part-time; M-F days)

This role has the possibility of also working at our Brooklyn Park Clinic or Hennepin County Jail. (Employees working at a level of 0.5 FTE and above are eligible for full medical/dental benefits, as well as several additional prorated benefits if less than full-time). Clinic hours are M - F during the day. Exact schedule TBD based on clinic need and provider preference.This position is responsible for evaluation and treatment of patients with acute and chronic physical, mental health, and substance use conditions, as well as providing preventative care services and education to patients and families within scope of practice and in collaboration with the supervising/collaborating physician.Note: Please include a cover letter with your application to be considered for this position.RESPONSIBILITIESProvide direct primary care services to adult patients as part of the primary care interdisciplinary team in the ambulatory departmentPerform initial and ongoing assessment of patients' physical, mental health, and substance use conditionsObtain and evaluate health and medical history, perform physical exam, conduct preventive screening procedures based on age and history, identify medical and health risksComplete accurate and timely documentation of observations, interventions, plan of care, referrals and follow-upFormulate appropriate differential diagnosis based on history, and physical examinationOrder, conduct, and interpret diagnostic tests, therapeutic treatments; may prescribe, administer and dispense therapeutic devices and medicationsMake appropriate referrals to other health professionals and community agenciesWork closely with interdisciplinary teams to provide the best care for the population servedMinimum QualificationsMaster's degree in Nursing or in Physician Assistant studies from an accredited programCurrent license (or eligibility) to practice as an Advanced Practice Registered Nurse or Physician Assistant in the State of MinnesotaCertification in the area of specialization by an agency authorized to certify professional nurses in Advanced Practice or Physician AssistantsKnowledge of electronic medical documentation software/systemsExperience working with a diverse and complex patient population including patients who have substance use disordersPreferred Qualifications2-3 years of NP or PA experience is preferredSALARYNurse Practitioner or Physician Assistant pay is based on years since licensure and specialty area.
