Job Title: Contingent Clinical Care LVN/LPN PT - MC Address: Lebanon, IN Job Description includes the responsibilities listed below: Administer oral, injectable, intravenous, and topical medications. Evaluate complex patient conditions, monitor pain levels, and document treatment responses. Perform routine assessments to monitor patient health and ensure overall well-being. Manage patient access to healthcare and connect individuals with community resources. Conduct medication reviews to identify potential side effects and drug interactions. Accurately document patient medical history using electronic health records. Keep an organized, well-stocked, clean clinic at all times. Critical Information Work schedules will be created in 6-week blocks following 3 weeks of training/orientation. Shifts will be between 7-10 hours during clinic operating hours, typically two or three weekdays and every other weekend. This is an in-person, patient-facing role. Education/Licenses Needed A full, active and unrestricted license as a Licensed Practical or Vocational Nurse in a State, Territory or Commonwealth of the United States.
