Responsibilities
- Provide direct patient care to residents in the LTC setting per individualized care plans
- Administer medications and treatments in accordance with state regulations and facility policy
- Monitor and document vital signs, assessments, and changes in resident condition
- Communicate promptly with RNs and the care team to escalate concerning findings
- Maintain accurate documentation in the charting system and MAR, ensuring entries are complete and timely
- Promote safety, infection control, and fall prevention; respond to resident needs and call lights
- Support admissions, discharges, and family education as part of the care team

Requirements
- Active Licensed Practical Nurse (LPN) license in West Virginia
- Point-Click-Care (PCC) experience required
- Ability to work in a long-term care/nursing home setting
- Traveler eligibility within 100 miles of the facility (distance limit)
- Willingness to participate in a two-week rotation schedule (2 days one week, 4 days the next)
