Experience: - Minimum 5 years of recent acute care Case Management experience in a hospital setting.

- Recent experience in an adult acute care hospital setting with discharge planning.

- Experience in a Level I, II, or III acute care hospital.

- Previous patient census of approximately 15-25 patients.

- Less than a 6-month gap since the last acute care Case Management role.

- Previous travel experience preferred.

License: - Active Virginia or Compact RN license required at the time of application.

Certifications: - BLS through the American Heart Association is required.

- CCM (Certified Case Manager) and ACM (Accredited Case Manager) are preferred.

Must-Have: - Proficiency in Epic is required.

- Acute care discharge planning and Utilization Review (UR) experience.

- Ability to effectively manage a high-volume, fast-paced patient population.

- Candidates with any license or background hits will not be considered.

Description: Client is located in a beautiful coastal region. It offers a paradise for outdoor lovers with hiking trails and various waterways. The Case Manager Registered Nurse will work weekdays from 8 AM to 4:30 PM, with some weekends and holidays. Longer assignments are available but not required. Candidates must work two of three winter holidays. Recent experience in acute care is essential. Knowledge of CMS regulations is necessary.
