Primary Care Nurse Practitioner / Physician Assistant About the Opportunity Our client, a growing community-based healthcare organization in Brooklyn, is seeking an experienced NP or PA to join its clinical team! This position will provide comprehensive primary care services to a diverse patient population while supporting preventive health and chronic disease management. The annual base salary range is $145,000 to $180,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer. Job Responsibilities Conduct patient evaluations, medical histories, and physical examinations Diagnose and manage acute and chronic medical conditions, including diabetes and other long-term illnesses Develop individualized treatment plans and medication management strategies Order and interpret diagnostic tests and laboratory results Provide patient education on disease prevention, treatment plans, and lifestyle management Coordinate care by referring patients to specialists when appropriate Collaborate closely with physicians, nurses, and clinical staff to ensure high-quality patient care Document patient encounters accurately in the electronic medical record Follow up with patients to monitor progress and adjust treatment plans as needed Participate in clinical meetings and quality improvement initiatives Maintain patient confidentiality and comply with health care regulations Job Requirements Graduate of an accredited NP or PA Active New York State Medical license Minimum 3 years of clinical experience as a Provider Experience managing chronic diseases such as diabetes, hypertension and other long-term conditions Comfortable working with diverse and underserved patient populations Strong communication and patient education skills Must speak Spanish Full-time or part-time opportunities available Flexible schedule with occasional weekend coverage (approximately once per month) #J-18808-Ljbffr
