This is a 13-week contract position.

Schedule

Monday through Friday, 7:30 AM to 4:00 PM. Every third weekend and holidays are required.

Job Description

We are seeking an experienced RN Case Manager to join our Care Management team. The ideal candidate will be responsible for discharge planning and care coordination for patients with high acuity and complex needs, including a significant behavioral health component.

Responsibilities

• Conduct comprehensive assessments of patients to devise individualized discharge plans.
• Coordinate care among multidisciplinary teams to foster optimal patient outcomes.
• Ensure high standards of patient care and adherence to clinical guidelines.
• Assist in managing complex cases involving behavioral health issues.
• Utilize electronic medical records systems such as EPIC, Wellsky, and Careport for documentation and tracking care.

Required Experience / Certifications / Licensure

• Minimum 5 years of hospital case management experience.
• 5+ years of acute adult care nursing experience.
• Active RN license in the state of Maine.
• Demonstrated ability in discharge planning and care coordination.
