PRN Physical Therapist (PT)Aventura at Walton Hills – Walton Hills, OH
Aventura at Walton Hills is seeking an enthusiastic and compassionate PRN Physical Therapist to join our rehab team. If you are looking for schedule flexibility, a collaborative therapy environment, and the opportunity to make a real difference in residents’ lives, this could be the perfect fit.

Provide skilled physical therapy services focused on mobility, balance, strength, and independence
Accurately document evaluations, treatments, and progress in Net Health and PCC
Collaborate with nursing, therapy, and interdisciplinary teams
Maintain compliance with all facility and regulatory guidelines
Current Ohio PT license
SNF/LTC experience preferred but not required
Experience with Net Health and PCC preferred
Strong communication and clinical skills
Compassionate, dependable, and team-focused mindset
