In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) - Rockingham County NH

Position Overview Signify Health, part of CVS Health®, is seeking a Part‑Time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations. You will engage with patients in their own homes, helping to simplify care by providing medication and medical history reviews, physical evaluations, and, when ordered, additional diagnostic tests such as diabetic eye exams and spirometry. Responsibilities Build trusted relationships and provide personalized health evaluations to patients in their homes. Conduct visits lasting about 45 minutes, using an iPad and an intuitive clinical workflow. Offer services including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening). Be flexible to travel locally and within licensed states as needed, and conduct virtual visits when required. Follow all Signify Health policies and procedures for patient care, documentation and safety. Work Environment As an in‑home clinician, your primary workplace is the private residence of health plan members. You must be able to safely perform duties in homes that may contain pets, secondhand smoke or other allergens. Qualifications Active, unrestricted license(s) in coverage area(s). Board certification (required). Multi‑state licenses (preferred). Ability to cover primary locations including Rockingham County. Compensation and Working Hours Anticipated weekly hours: 29.5 hours (part‑time). Pay is based on a base hourly rate or full‑time equivalent salary, ranging from $41.85 to $90.13 per hour, depending on experience, education and geography. Additional reimbursement may be available for completed Diagnostic and Preventive Services.
