Job Description: Registered Nurse - Case Manager
Facility: UChicago Medicine Ingalls Memorial Hospital
Location: Harvey, IL
Contract Duration: 6 weeks
Start Date: 04/20/2026
Shift: Day, 5x8-hour (08:00 - 16:30)



Key Responsibilities
Conduct initial patient/family assessments within 48 hours of admission.
Complete Care Coordination Assessments to identify intervention needs.
Document patient status, treatment plans, goals, and follow-ups in medical charts weekly or as situations change.
Participate in multidisciplinary rounds and collaborate with staff regarding patient care.
Communicate clinical updates to patients, families, and care teams.
Evaluate treatment plan effectiveness and adjust as needed.
Review and document final discharge plans with patients/families.
Maintain accurate records of discharges and referrals.



Required Qualifications
Active Illinois RN license in hand.
3-5 years of RN experience (acute hospital or case management preferred).
ACM or CCM certification preferred.
BLS certification require
