We're looking for a Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), or Licensed Professional Counselor (LPC) to join our collaborative Behavioral Health team.

This is more than a therapy role-it's an opportunity to be part of an integrated Primary Care team that treats the whole person. We recognize the powerful connection between physical, emotional, and behavioral health, and we work together to ensure our patients receive truly comprehensive care. This position will serve patients based within a public school setting AND a Clinical Setting at our Ava Clinic location. 

What You'll Do

As a key member of our care team, this position will offer comprehensive behavioral health services through traditional therapy in a clinical setting and in a school setting with children, adolescents, and adults in individual and group environments. The provider will be responsible for:

• Provide traditional outpatient therapy to children, adolescents, and adults
• Facilitate individual and group counseling sessions
• Conduct initial and follow-up assessments
• Develop and implement individualized treatment plans
• Provide crisis intervention as needed
• Advocate for patients and families within the healthcare system
• Collaborate closely with physicians and multidisciplinary team members
• Maintain clear, timely clinical documentation
• Occasionally travel to local sites as requested

What We're Looking For

• • Master's degree in Social Work or Counseling from an accredited institution.
  • Current state licensure as an LCSW, LMSW, or LPC .
  • Working knowledge of the DSM-5.
  • Strong knowledge of behavioral health practices and therapeutic techniques.
  • Experience working with children and adolescents preferred.
  • Exceptional communication and interpersonal skills when working with patients, families, physicians, and healthcare team members.
  • Ability to build collaborative relationships and work effectively within a multidisciplinary care team.
  • Ability to work independently while contributing to a team-oriented environment.
  • Strong organizational and time management skills.
  • Ability to maintain clear, accurate, and timely clinical documentation.
  • Willingness and ability to travel to local sites as needed.
  • Ability to clearly and concisely document sessions and familiarity with healthcare regulations.
  • Experience in school-based or integrated behavioral health settings preferred.
