Physical Therapist – Bourne, MA Overview FOX Rehabilitation is expanding in Bourne, MA, and we are seeking licensed Physical Therapists to provide one‑on‑one care to older adults in private residences or senior living communities. The role emphasizes autonomy, flexibility, and meaningful patient outcomes, with the opportunity to integrate telehealth into the schedule. Responsibilities • Provide individualized physical therapy to older adults at home or in senior living settings, including independent living, assisted living, and memory care.• Conduct evaluations, develop care plans, and deliver evidence‑based treatment sessions.• Document care accurately in FOX’s EMR system to meet regulatory standards.• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and fitness specialists.• Educate patients, caregivers, and senior living staff on safety, function, and aging in place.• Offer telehealth or virtual care when appropriate to extend access and flexibility. Qualifications Valid Physical Therapy license in the state of practice or eligibility to obtain one. Degree from an accredited Physical Therapy program. Basic computer and EMR proficiency. Current CPR certification. Ability to meet the physical demands of in‑home and community care (e.g., lifting, transfers, standing, walking, driving). Physical Requirements Exert medium physical effort: lift 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly. Perform CPR and provide moderate to maximum assistance with patient transfers and bed mobility. Reposition patients, stand/walk for extended periods, climb stairs, and perform therapeutic activities. Drive to patient homes and senior living communities as part of the Geriatric House Calls model. Wear PPE when required.
