Job Summary and Qualifications

The Nurse Navigator is responsible for the programs and resources that support the navigation of patients through complex healthcare environments for the optimization of care and outcomes. S/he facilitates patient follow-up through coaching, information transfer, and communication in order to improve continuity of care, enhance the medical management of complex disease, and streamline the intervention process. 

Duties as a Nurse Navigator will include:  

You will guide patients through the health care system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes.

Your role serves as a point of contact for referring physicians, patients and caregivers to provide resources and assistance with accessing clinical and supportive care services. 

You will review patient case lists generated by technology-enabled data triggers to identify patients that could benefit from intervention.  

Your role maintains detailed records to measure effectiveness of program and suggest ways to improve throughout the process, as well as tracks outcomes of referrals and treatments.  

You will review medications, educate patients as needed, and initiate communication regarding dashboard triggers with care team.  

 

Qualifications for a Nurse Navigator: 

 

Current licensure in the State of Kansas as a Registered Nurse (RN) or Current Multi-state licensure as a Registered Nurse 

Current Basic Life Support (BLS) course through the American Heart Assoc. or American Red Cross – Healthcare Provider CPR 

Graduate from an accredited school of nursing – BSN preferred 

Experience as a nurse navigator and/or patient educator preferred 

RN experience in the ICU and/or Med/Surg preferred
