Enjoy working in a patient-centered medical home (PCMH) model of care which allows you as a provider to focus on what you do best, taking care of patients! As part of our team there is a full complement of physicians, mid-level providers, pediatricians, dental, providers and clinical staff to support patient care. Our organization uses NextGen as its electronic health record system. Case Management, Diabetic Education, 340-B Pharmacy and Patient Assistance Programs are in place to support the needs of patients. Overview of Duties:
Perform annual wellness visits and health assessments on a population with chronic conditions
Provide patient health education opportunities
Assist in closing quality care gaps (i.e. screenings and labs)
An enthusiastic collaborator contributing to the enhancement of care delivery
