Patient Navigator - Oncology Clinic (Oak Lawn)

Job Summary The Patient Navigation position offers individualized assistance to patients, families and caregivers to overcome healthcare barriers. Patient Navigators work in a variety of settings—community, hospital, home, primary care, and tertiary care. They prioritize making patients feel comfortable and guided throughout their journey. Location Advocate Medical Group (AMG) is part of Advocate Health. The role will be based at Advocate Christ Medical Center Hematology/Oncology in Oak Lawn, IL, delivering comprehensive cancer care across South Chicagoland. Qualifications Bachelor’s degree in a healthcare related field preferred 2+ years of experience working in a medical setting with oncology patients and families Ability to prioritize workload and work independently Responsibilities Schedule: Monday – Friday, 8 a.m. – 4:30 p.m. As a Patient Navigator, you will support and connect with patients, survivors, families and healthcare providers to ensure a seamless, compassionate experience. Coordinate appointments with providers for timely delivery of diagnostic and treatment services. Monitor patient satisfaction and work with teams to address any identified issues or barriers. Advocate on behalf of the patient and empower self‑advocacy. Ensure appropriate medical records are available at scheduled appointments. Arrange transportation assistance with available resources. Physical Requirements & Working Conditions Ability to work independently. Ability to handle stress and high workload volumes. Ability to communicate denial of services to members and providers. Lifting up to 35 pounds without assistance; for heavier lifts, patient handling equipment must be used. Compensation and Benefits Pay Range: $24.10 – $36.15 per hour, based on qualifications, skills and experience. Base compensation within the range. Premium pay such as shift or on‑call compensation.
