Job Summary:

We're seeking a compassionate and skilled LPN to join our team of healthcare professionals in Raleigh, NC. As an LPN at Litchford Health & Rehabilitation Center, you'll have the opportunity to make a meaningful difference in the lives of our patients, working closely with our multidisciplinary team to provide personalized care and support.

Responsibilities:

• Provide direct patient care, including administering medications, monitoring vital signs, and performing treatments as prescribed by physicians

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Conduct vital sign checks, report any changes or concerns to the nursing team, and document patient information accurately and thoroughly

• Assist with daily living activities, such as bathing, dressing, and grooming, as needed

• Participate in quality improvement initiatives and contribute to a positive and respectful work environment

• Maintain patient confidentiality and adhere to HIPAA guidelines

• Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care

Requirements:

• Current LPN licensure in the State of North Carolina

• High school diploma or equivalent required
