Nurse Practitioner or Physician Assistant (PRN) - In-Home Health Assessments

Our national network of clinicians meets patients at home, helping older adults and other at‑risk individuals enjoy better quality of care and improved health outcomes. $2,000 Sign‑On Bonus Responsibilities During a visit that can last up to one hour, providers review and observe a member’s current health, medical history, medication adherence, social environment, and other risks. The Matrix Comprehensive Health Assessment (CHA) helps to close multiple care gaps with a single visit. Conduct adult/geriatric assessments to include medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow‑up, and clear documentation according to Matrix guidelines and protocols. Administer and order point of care and lab‑based testing as needed. Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate. Collaborate with Primary Care Physician (PCP) on patient education and follow‑up as necessary. Provide services at the member’s place of residence, which can be in their homes or Skilled Nursing Facilities. Benefits Flexibility – Visits are scheduled based on your availability. We conduct assessments Sunday–Saturday from 8 am–7 pm, allowing you to work during the day, evening, and/or weekend. Competitive Pay – Earn income based on the number of assessments you complete. Monthly averages of 20–100 visits can earn $1,800–$11,500, based on a per‑assessment rate of $90–$115, depending on your state of residence. Mileage reimbursement, 401(k), employee referral bonuses, and limited voluntary benefits. Comprehensive onboarding training, a preceptor, 1:1 feedback, and free access to continuing education. Support for obtaining additional state licensure and credentialing in neighboring states if needed. Full‑time opportunities for PRN providers who wish to transition to a full‑time role and receive a sign‑on bonus. Qualifications Master’s Degree or commensurate experience and satisfactory completion of NP licensure. Current NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority as needed. AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty. Current BLS, ACLS, or CPR certification. One year of NP experience preferred; new graduates encouraged to apply. Valid state driver’s license for the state of residence. Automobile insurance coverage required for the state of residence (or ability to obtain). Bodily injury: $100,000 per person / $300,000 per incident. Property damage: $50,000. Collision and comprehensive deductibles: $1,000 or less. Strong computer skills and familiarity with employee health/medical record software. Excellent verbal and written communication skills.
