Pharmacist - Rehab

We are seeking a dedicated and skilled Pharmacist to join our inpatient rehabilitation team. This role is crucial in providing pharmaceutical care to patients undergoing rehabilitation following various medical conditions.

Responsibilities

• Manage and oversee medication distribution and ensure safe and effective medication management for all patients.
• Collaborate effectively with healthcare providers to optimize pharmacotherapy and improve patient outcomes.
• Conduct medication reconciliation and provide education to patients and their families regarding prescribed medications.
• Monitor patient responses to medications and adjust therapies as necessary based on individual patient needs.
• Document all relevant clinical information and interventions in the electronic health record.

Qualifications

• Must be licensed to practice as a Pharmacist in Wisconsin.
• Minimum of 1 year of experience in an acute care or inpatient rehabilitation setting.
• Working knowledge of electronic health record systems, specifically EPIC, and automated medication management systems like Omnicell.
• Strong communication and interpersonal skills, with the ability to work collaboratively within a multidisciplinary team.
• Detail-oriented with effective problem-solving abilities.
