Schedule : Full Time | 40 hours per week | 8:00am -4:30pm | Weekend & Holiday Rotations, Required
Department: Thome PACE - Jackson, MI | Day Center & In-home Care

Travel Requirements: This role involves regular travel between Jackson, Lenawee and Hillsdale counties providing Home Based Care.

No schedule mandation. Mileage reimbursement offered. Great work life balance!

What You’ll Do:

As a Certified Nurse Aide (CNA) in our Day Center and providing in-home care, you'll play a vital role in ensuring our participants receive the highest quality care. From assisting with daily activities to providing comfort and companionship, your work will make a meaningful impact on the lives of those we serve in Jackson, Lenawee and Hillsdale counties.

Key Responsibilities

• Provide Hands-On Care – Assist with personal care (ADLS) like bathing, grooming, dressing, and toileting.
• Support Mealtime– Help participants enjoy their meals and stay hydrated for good health.
• Keep Things Moving – Support mobility, including turning, positioning, and safe transfers.
• Perform Vital Checks – Measure and record vital signs, weight, and intake/output.
• Maintain Comfort & Care – Keep rooms tidy, cozy, and well-stocked with essentials.
• Provide Responsive Support – Respond with kindness and compassion.
• Monitor with Watchful Eyes – Report any changes in conditions to the leadership ASAP.
• Demonstrate Respect & Professionalism – Maintain confidentiality while delivering top-notch care.

On-Call and Flexibility:

• On-Call Responsibilities: Participate in after-hour on-call rotations, including evenings, weekends, and holidays. On-call pay premium offered.
• Rotation Schedule: Be available for evening and weekend shifts as part of the rotation.

Skills, Knowledge and Expertise

• A current and valid Michigan CNA Certification, required.
• CPR Certified or willing to optain.
• Six (6) months of clinical experience as a licensed CNA, preferred. 
• Prior home care experience, preferred.
• The ability to read, write, understand, and follow directions.
• A heart for helping others and the physical ability to lift, stand, bend, transfer, stretch, walk, push, and pull as needed.
• Ability to lift a minimum of 35 pounds frequently is required
• Must be 18 years of age or older.
