Role Description This is a part‑time, on‑site Case Manager role based in Oceanside, CA. The Case Manager will coordinate client care, including intake, assessments, and ongoing case monitoring, to support individualized treatment plans. Day‑to‑day responsibilities include maintaining accurate documentation, tracking progress, and collaborating closely with therapists, clients, and external providers to ensure continuity of care. The role involves connecting clients with community resources, benefits, and supportive services that align with their clinical and practical needs. The Case Manager will also participate in team meetings, contribute to care planning, and uphold confidentiality, ethical standards, and regulatory requirements. Qualifications Client care and case management skills, including intake, assessment, care coordination, and follow‑up. Strong communication, interpersonal, and relationship‑building skills with diverse client populations, including veterans and first responders. Organizational skills for managing caseloads, documentation, scheduling, and timely record‑keeping. Knowledge of community resources, social services, and referral processes relevant to mental health and family therapy settings. Ability to collaborate effectively in a multidisciplinary team and maintain professional boundaries and ethical standards. Familiarity with trauma‑informed care principles and evidence‑based behavioral health practices. Proficiency with electronic health records (EHR) or case management systems and basic office software. Relevant education in social work, psychology, counseling, or a related field; licensure or certification in case management or behavioral health is preferred. Experience working in mental health, substance use treatment, or family therapy environments is highly beneficial.
