Successful candidates will have experience providing supervision and quality assurance reviews, case management, completing assessments and individual service plan development, and have good verbal and written communication skills, team leadership and facilitation skills, and organizational skills to meet documentation and productivity targets while utilizing critical thinking to enhance successful outcomes for individuals with developmental disabilities, including those with co-occurring disorders.Come join a team that values creativity, quality service, integrity, accountability, and teamwork. You will perform difficult professional work designed to provide case management services, including conducting intakes, eligibility determination, needs assessment, and service plan development. This will include the following:Linking, monitoring, referring, coordinating, and facilitating services needed by the individuals.Conducting and utilizing assessments for individual service plan developmentCoordinate and facilitate individuals' treatment team meetingsImplement individual service plans and revise the service plan as needed/requiredMonitoring quality of care through home and community visits to ensure health and safety risks are addressedDocumenting all services, contacts, and attempted contacts with individuals and team membersWrite individual service plans, progress notes, and quarterly service plan reviewsMaintain annual trainings and provide in-service trainingsCoordinate and lead interdisciplinary team meetings to develop and monitor service plans and delivery of services including service authorizations and private vendor authorizationsAnalyze data and prepare administrative reportsEnsure proper adherence to federal, state, local, and agency regulations and licensure standardsLink individuals with appropriate community resources and advocate for individuals in the community, including completing service plans, CANS, and presentation at FAPT/FAST meetingsProvide consultation as emergencies arise and crisis intervention/supportive counseling for individuals and their families/community providersServe on CS program committees with providers and/or administratorsOrganize and maintain individuals' electronic health records to include updating data for the performance contract with DBHDS and dashboard measuresDevelop cooperative relationships with community groups and agenciesMaintain ongoing communication with families, vendors, landlords, county programs and agencies, and state agencies, including care coordination agenciesProvide supervision, monitoring, and evaluation of direct supports' productivity, work quality, and documentation timeliness, and assign cases.Complete performance evaluations and implement performance improvement plans.Provide case consultation to Clinical Services CaseworkerComplete utilization reviews and assist staff in prioritizing workloadsUpdate status on waitlists, including the statewide waiver waitlist; complete SARFs and make referrals for services as neededRequirements:Bachelor's degree in psychology, social work, counseling, or a related behavioral science field.3 years of experience providing service coordination and case management to individuals with developmental disabilities.Preferences:1 year of supervisory experienceExperience providing services through Medicaid State Plan Option and Developmental Disability Waivers.Knowledge of person-centered practices, developmental disabilities, and dual diagnoses (DD/MH).Experience using electronic health records; Credible preferred.Experience using WaMS for DD Waiver recipientsSpecial Requirements:Must possess a valid driver's license; Must pass DMV record check.Must pass TB test.Offer of employment contingent upon the candidate passing a fingerprint-based national criminal history record check.Child Protective Services Requirements: Effective 1999, mandated by the Department of Behavioral Health and Developmental Services 12VA35-105-400 rules and Regulations for Licensing Providers by the Department of Behavioral Health and Developmental Services related to Criminal Registry Checks, a search of the registry of founded complaints of child abuse and neglect maintained by the Department of Social Services.Work Schedule: This is a full-time position, based in our Woodbridge office, requires a 5–day work week and typical work hours are 8:30 AM – 5 PM.Starting Salary Range: $82,777.50 - $111,033.00 AnnuallyPrince William County Community Services maintains a drug-free workplace.We also offer great benefits, including:Retirement from the Virginia Retirement System (VRS)401a and 457 retirement savings and investment plansPaid Annual LeavePaid Personal LeavePaid Sick LeavePaid HolidaysOptional Group Medical and Dental Health PlansOptional Group Life InsuranceAn Employee Assistance Program (EAP)Career Development OpportunitiesFull-time positions with Prince William County Government qualify for Public Service Loan Forgiveness.
