Job Title: ADRC Assessor (Licensed Social Worker) Location: Greater Cincinnati / Southwestern Ohio (Hybrid/Remote with Community Visits) Job Type: Full-Time, Non-Exempt Pay Band: $45,000 - $65,000 Your Advocacy. At Council on Aging of Southwestern Ohio (COA), Licensed Social Workers serve as advocates, navigators, and problem-solvers for older adults and individuals with disabilities. As an Aging and Disability Resource Center (ADRC) Assessor, you’ll use your social work training to assess needs, address safety concerns, and connect individuals and families to the services that allow them to live with dignity and independence. Job Summary The ADRC Assessor role is ideal for Licensed Social Workers who want to work at the intersection of clinical assessment, systems navigation, and community-based advocacy. You’ll complete person-centered assessments in homes and community settings, identify risks and strengths, and help individuals understand and access long-term services and supports. You’ll collaborate with internal teams, care coordinators, and community partners while maintaining a strong focus on ethics, autonomy, and client-centered decision-making. What You’ll Do Conduct face-to-face, person-centered assessments with individuals and caregivers in homes and community settings Evaluate psychosocial, environmental, and health-related factors that impact safety and independence Identify health and safety risks and advocate for appropriate interventions and supports Develop initial plans of care that reflect client goals, strengths, and preferences Provide education to individuals, families, legal guardians, and authorized representatives Connect individuals to community resources, benefits, and long-term care options Support discharge planning and transitions from hospitals or facilities to community-based living Document assessments, eligibility determinations, and care plans in required systems Collaborate with care coordination teams and Managed Care Organizations as appropriate Uphold confidentiality, HIPAA standards, and ethical practice expectations Participate in team meetings, case conferences, and continuous improvement initiatives Qualifications Licensed Social Worker (LSW) in good standing At least 1 year of experience in social work, case management, medical social work, aging services, or a related field Strong understanding of person-in-environment, family systems, and community-based services Experience assessing psychosocial needs, risk factors, and protective supports Knowledge of aging, disability, and long-term services and supports preferred Excellent interviewing, documentation, and advocacy skills Ability to work independently while contributing to a collaborative, mission-driven team Strong written and verbal communication skills Proficiency with Microsoft Word, Excel, and electronic documentation systems Valid driver’s license and reliable transportation for travel within COA’s five-county service area Ability to work from a confidential home office with high-speed internet Newer LSWs with strong internship or field experience in aging, healthcare, or community-based services within Southwestern Ohio are encouraged to apply.
