About This Role The Utilization Management RN is responsible for coordinating the complex care needs of patients, serving as the primary contact for service access, diagnostics, treatment activities, and multidisciplinary collaboration. Highlights Pay: $41.72 - $105.65/Hourly Location: Boston, MA Facility: The General Hospital Corporation Schedule: Per Diem Department/Unit: The General Hospital Corporation What You'll Do Collaborate with the interdisciplinary team to create tailored health care plans for assigned patients. Develop individualized plans with the MD for appropriate services for the patient. Report pertinent observations and reactions regarding clients to the appropriate person. Assess and coordinate patient's discharge planning needs with members of the healthcare team. Resolve patient problems and needs by utilizing multidisciplinary team strategies. Educate patients on navigating throughout the health system. Requirements Education: Bachelor's Degree in Nursing required, Master's Degree in Nursing preferred Experience: 2-3 years in an acute care setting and/or provider practice and/or community-based role with discharge/transitional planning background preferred Licenses & Certifications: MA Registered Nurse License required About Mass General Brigham Mass General Brigham is a leading healthcare provider committed to delivering exceptional patient care.
