Hospital Liaison Qualifications: 

• 2–3 years of Hospital Liaison, healthcare marketing, or skilled nursing experience required.

• Experience working directly with hospital social workers, case managers, and discharge planners.

• Strong understanding of hospital referrals and the skilled nursing admission process.

• Established hospital relationships are a plus.

• Strong communication, follow-up, and relationship-building skills.

• Goal-driven with a focus on growing referrals and admissions.

Hospital Liaison Job Overview: 

The Hospital Liaison is responsible for developing and cultivating new patient prospects for admission to the facility. The incumbent will be responsible for maintaining relations with community partners, family members, and facility personnel.

Hospital Liaison Responsibilities:

• Visit assigned hospitals regularly and build strong referral relationships.

• Follow up with social workers, case managers, patients, and families.

• Track referrals from initial contact through admission.

• Work closely with Central Intake and facility teams to coordinate admissions.

• Market our skilled nursing services and increase hospital referrals.

• Maintain consistent communication with referral sources.
