RN Case Manager — Geriatric Care Coordination (Contract)

Pride Health is seeking an RN Case Manager to travel between two Skilled Nursing Facilities in New London County, CT. The schedule is Monday to Friday, 8am–5pm, on a 3-4 month contract with a pay rate of $43-46/hour. The role requires an active RN license, BLS certification, and at least 1 year of clinical experience in a long-term care setting.
