Job Summary:
Home Health Registered Nurse (RN) Portland, Oregon. This position involves providing skilled home health nursing services within the West Portland area and surrounding communities. The role requires delivering patient care independently in a home setting, following clinical protocols, and coordinating with interdisciplinary teams to ensure optimal patient outcomes.

Job Details:

• Schedule: 8-hour days shifts, minimum of 40 hours per week, start date on 08/03/2026.
• Required Qualifications: Oregon State Licensure, BLS certification, 1 year of skilled home health experience. First-time travelers accepted.
• Preferred Qualifications: Experience with Oasis documentation, adult home health experience.
• Certifications: BLS required.
• Additional Information:
  • Must have reliable transportation; occasional maximum radius of roughly 60 miles per day is expected.
  • COVID-19 vaccination confirmed with primary series and one booster.
  • Traveler will be working in a home health environment using EPIC, Rover, and Oasis systems.
  • No on-call required.
  • No float between units.
  • Expectation to meet productivity points with up to six patient visits per day.
  • Daily charting is done via electronic systems; comfortable with wound care, medication management, and adult health assessments.
  • Open to local candidates within a 50-75 mile radius.
  • Submit references covering 1 year, with approval prior to start.
  • Additional modules are completed prior to starting and annually, self-reported in Workramp.
