Care Manager - Senior Living
LiveWell Homecare Agency, LLC
Role: Lead Caregiver in senior care, serving as a shift leader to ensure high-quality resident care and team support.
Responsibilities: Lead shift assignments, manage attendance, coordinate coverage, provide hands-on ADL support, coach caregivers on quality and documentation standards, review charting to align with Individualized Service Plans (ISP), monitor resident changes, communicate concerns promptly, support onboarding, mentor new team members, and promote safety and compliance during shift transitions.
Qualifications & Skills: High School Diploma/GED, CPR, First Aid, Medication Administration certifications, strong communication, delegation, and decision-making skills, and experience in assisted living, memory care, or senior care.
Key Duties: Model professionalism, accountability, and compassionate service while leading and supporting caregiving staff to deliver exceptional resident care.
