Job Title: Physical Therapist

Work Location: Outpatient

Work Week Hours: 40 hours per week

Shift: Weekdays 0700 to 1530 OR 0800 to 1630

Weekends/On call - None

Number of Positions: 1

Required: Maine License

Start Date: ASAP

EMR: CPSI

Immunizations Required:

• MMR x 2 vaccination or a Positive Titer
• Varicella x 2 vaccination or a Positive Titer
• QuantiFERON x 1 within 1-year
• Hepatitis B series, or Positive Titer, or signed Declination
• Flu Vaccination between 10/1 to 6/30 or a Bonafide Medical Exemption
