THE POSITION

The Department of Health (DOH) is seeking a Community Health Nurse who is ready to make a difference in Berks County and surrounding communities. In this role, you will help assess health needs, support disease prevention efforts, and connect people with important services. DESCRIPTION OF WORK

This position supports public health efforts in the Southeast District by assessing community needs, providing direct services, and collaborating with local partners. As a Community Health Nurse, you will perform the following duties:

• Health Assessment: Analyze county-level data to identify public health needs and guide education and clinical activities
• Community Liaison: Work with community partners to link program resources and support local public health infrastructure
• Program Development: Create and support public health initiatives that address identified health priorities
• Disease Monitoring: Track vaccine-preventable diseases and apply surveillance systems to support improved health outcomes in the assigned area
• Health Education: Develop education and outreach on important public health topics to support community wellness
• Clinical Services: Provide immunizations, testing, and other clinical services to prevent and control disease

Work Schedule and Additional Information:

• Full-time employment, 37.5 hours per week
• Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.
• Telework: You will not have the option to telework in this position.
• Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Special Requirements:

• You must be in possession of active authorization to practice as a Registered Nurse in Pennsylvania.
• All employees possessing an active temporary practice permit must obtain licensure as a Registered Nurse within the one (1) year period defined by the Pennsylvania State Board of Nursing.

• You must be able to perform essential job functions.

Legal Requirements:

• You must pass a background investigation.
• This position falls under the provisions of the Child Protective Services Law.
  • Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
