Care Manager - Emergency Department (ED)
Location: Bronx, NY
Job Type: Contract
"Navitas Healthcare, LLC" is seeking an experienced Care Manager - ED to support care coordination, utilization review, and discharge planning activities within a hospital emergency department setting. The ideal candidate will have strong clinical nursing experience and excellent communication and organizational skills.
Key Responsibilities- Coordinate patient care and discharge planning activities.
- Perform utilization review and care management functions.
- Collaborate with interdisciplinary healthcare teams to ensure appropriate patient care transitions.
- Maintain accurate documentation and compliance with hospital policies.
- Support patient flow and resource utilization within the Emergency Department.
- Utilize computer systems and electronic medical records effectively.
- Minimum 5 years of clinical experience as a Registered Nurse (RN) .
- Experience with:
- Care Coordination
- Utilization Review
- Discharge Planning
- Computer literacy and proficiency with healthcare documentation systems.
- Associate Degree in Nursing (ASN/ADN) from an accredited nursing program.
- Active New York State Registered Nurse (RN) License .
- BLS Certification .
- PRI and Screen Certification (or ability to obtain within three months of employment).
- Primary Source Verification of RN License and Registration through NYSED.
- Bachelor's or Master's Degree in Nursing.
- Leadership experience.
- Case Management Certification.
- 35 Hours per Week
- 7 Hours per Day
- 5 Days per Week
