About the Role:

We're seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. As a key member of our clinical team, you'll build strong relationships with the facility, maintain a patient-centered approach, and be a collaborative team player.

Responsibilities:

• Provide high-quality preventative care to residents in skilled nursing facilities through on-site visits and remote work from home

• Conduct patient assessments, develop treatment plans, and provide education to patients and families

• Collaborate with facility staff to ensure seamless care transitions and comprehensive patient care

• Monitor patient progress, identify trends, and adjust care plans accordingly

• Maintain accurate and timely documentation, supported by a dedicated scribe team

Requirements:

• Active NP or PA license (in the state of practice)

• 1+ year of experience in:

  • Geriatric care

  • Documentation review

• Excellent communication, collaboration, and teamwork skills
