Position Summary: The Long-Term Acute Care Registered Nurse (LTAC RN) provides specialized care for patients with serious medical conditions who require extended hospital stays for treatment and recovery. These patients often have complex medical needs, including ventilator support, wound care, and management of chronic conditions. The LTAC RN is responsible for delivering high-quality nursing care, managing patient assessments, administering treatments, and coordinating care within a multidisciplinary team to optimize patient outcomes in an LTAC setting.

Key Responsibilities:

• Provide comprehensive, patient-centered care to individuals with complex, multi-system illnesses or injuries in the LTAC environment.
• Conduct thorough patient assessments, monitor vital signs, lab results, and clinical conditions, making necessary adjustments to care plans based on patient status.
• Administer medications, intravenous fluids, and treatments as prescribed by physicians and in accordance with established protocols.
• Manage ventilator-dependent patients, including monitoring and adjusting settings, providing respiratory care, and collaborating with respiratory therapists.
• Perform advanced wound care, including dressing changes, and manage complex wound healing, pressure ulcers, and surgical sites.
• Collaborate with physicians, respiratory therapists, physical therapists, and other members of the healthcare team to develop and implement individualized care plans.
• Educate patients and families on health management, treatments, rehabilitation goals, and discharge planning.
• Monitor patient progress, identify potential complications, and intervene as necessary to prevent further deterioration.
• Provide emotional support to patients and families, addressing concerns, and facilitating communication between healthcare teams and families.
• Document all patient care activities, assessments, treatments, and progress accurately and in a timely manner.
• Ensure adherence to infection control practices, safety protocols, and hospital policies.
• Participate in quality improvement initiatives and attend mandatory training and continuing education programs to stay current with best practices in LTAC nursing.

Qualifications:

• Current Registered Nurse (RN) license in the state of employment.
• Associate's or Bachelor's degree in Nursing (ADN or BSN).
• CPR, ACLS (Advanced Cardiovascular Life Support), and BLS (Basic Life Support) certifications required.
• Minimum of 1-2 years of nursing experience in an acute care, ICU, or LTAC setting preferred.
• Experience with ventilator management, critical care, and advanced wound care is highly desirable.
• Strong clinical skills with the ability to manage complex medical conditions, including multi-organ failure, respiratory support, and infection control.
• Ability to collaborate effectively within an interdisciplinary team and communicate with patients and families clearly.
• Compassionate, patient-focused care approach with a strong emphasis on comfort and support for critically ill patients.
• Strong critical thinking, problem-solving, and organizational skills.

Physical Demands:

• Ability to lift, transfer, and assist patients with mobility and positioning.
• Stamina to work long shifts and manage the physical demands of the role.
• Ability to manage stressful and emotionally challenging situations related to patient care.

Working Conditions:

• Work is performed in a hospital or LTAC facility setting, which may involve exposure to infectious diseases, hazardous materials, and physical demands.
• Exposure to emotional challenges, particularly related to patients with long-term illnesses and their families.
