Description
Responsible for delivering case management services for clients and families as related to Mercer Community Services Food Pantry. ESSENTIAL FUNCTIONS:
- Assesses client needs; investigate their eligibility for services and designs, implements and monitors individual program plans for clients.
- Aids individuals and families requiring agency assistance.
- Maintains client records and completes required documentation according to agency, accreditation, and funder standards. This may include, but is not limited to data entry, progress notes, activity reports, time sheets and logs in program-approved database as well as hard copy files.
- Connects clients to referral entities and resources (housing, job training, counseling, housing, medical services, etc.)
- Networks with other providers to generate community resources for participants.
- May provide education on budgeting, credit repair, vocational development in individual and group settings.
- May transport consumers to appointments or meet consumers in the field, or at remote pantry locations etc.
- Ordering food products for food pantry. Maintaining Oasis database for charting clients receiving food. Recording volunteer hours and reporting.
- May assist with pantry operations (stocking shelves and receiving consumers).
- Other duties as assigned to support pantry operations and pantry consumer needs.
- High School / GED + 5 years of human services experience; OR
- Relevant professional credential such as CPRP, CRC, CADC + 2 years related exp., OR
- A combination of one or more years of college + related work or life experience together equaling four years; OR
- Bachelor's degree in human services field, rehabilitation, nursing, or a related discipline.
