We are actively hiring a Physical Therapist (PT) to join our rehab department at Dawn Hill Home in Bristol, RI. This is an excellent opportunity for a therapist who enjoys working in a skilled nursing environment and values a collaborative, patient-centered approach to care..

• Evaluate residents and develop individualized physical therapy treatment plans

• Provide skilled therapy interventions focused on mobility, strength, balance, endurance, pain management, and functional independence

• Supervise and collaborate with PTA staff as appropriate

• Complete accurate and timely documentation in accordance with regulatory and facility standards

• Collaborate with nursing, rehab, and interdisciplinary teams to ensure continuity of care

• Educate residents, families, and staff regarding mobility, safety, transfers, and therapeutic exercises

• Participate in discharge planning and recommend appropriate equipment and services to support safe transitions

• Graduate of an accredited Physical Therapy program

• Active Rhode Island Physical Therapist license (or eligibility)

• Long-term care or geriatric experience preferred (not required)

• Strong clinical, communication, and teamwork skills

• Passion for resident-centered care and functional outcomes
