Description:
Home Health position available.
The traveler's permanent address must be 50 miles outside of the home unit.
Schedule includes 6 visits a day without a max mileage limit.
The average mileage driven is approximately 60-80 miles a day, with mileage reimbursed weekly.
Candidates must have a minimum of 1 year of Home Health experience.
First-time travelers will be accepted if they are seasoned in home health.
This position mostly covers Northern areas with a focus on diagnoses including chronic diseases, orthopedics, and post-surgical cases.
Patient types may include chronic conditions, orthopedic surgeries, falls, weaknesses, CHF, COPD, and occasionally stroke patients.
Candidates must hold a valid driver's license and proof of insurance.
Skills required include gait training, balance training, and non-pharmaceutical pain management.
The role involves facilitating a plan of care to achieve the highest level of functioning efficiently.
Good communication with the clinical team is essential.
Recognition of signs and symptoms to prevent readmissions is necessary.
Experience with Homecare Homebase is required.
Department support includes leadership, scheduling, and clinical management.
Technology used includes EPIC Rover, Samsung note 9 phones, and Focura for document uploads.
The departmental culture emphasizes a team environment.
