Position: Nurse (Stroke) Navigator

Shift: Day 5x8-Hour (08:00 - 16:30)

Job Description

The Nurse (Stroke) Navigator is a professional Registered Nurse (RN) who provides individualized guidance, education, coordination, and support to stroke patients, families, and caregivers across the continuum of care. This role is integral to a Certified Comprehensive Stroke Center, utilizing specialized knowledge in stroke care to improve patient outcomes and reduce barriers to care.

Responsibilities

• Provide comprehensive inpatient stroke education to patients and families.
• Reinforce stroke prevention strategies, risk factor modification, medication adherence, and recognition of stroke warning signs.
• Lead and participate in community outreach initiatives focused on stroke awareness, prevention, education, and resource access.
• Participate in the development, implementation, monitoring, and sustainability of stroke program initiatives and quality improvement activities.
• Collaborate with inpatient, outpatient, rehabilitation, and community partners to improve transitions of care and support evidence-based stroke management.

Qualifications

• Current RN Licensure and Certifications: NIHSS, CA RN License, and BLS (Basic Life Support) required.
• Preferred: Certified Neuroscience Registered Nurse (CNRN) or Stroke Certified Registered Nurse (SCRN) certification.
• Minimum of 5 years of recent relevant acute care experience with stroke patients or as a nurse navigator assisting patients with post-discharge barriers.
• Effective communication skills related to stroke education for both staff and patients.
• Proficiency in Microsoft Office software, including Excel and Word.
• Knowledge of medical terminology and third-party reimbursement processes is beneficial.

Skills and Knowledge

• Ability to ensure follow-through with responsibilities and communicate effectively with various levels of healthcare staff.
• Experience with chart review to identify gaps in stroke care and collaborate with primary nurses.

Unit Information

This position does not involve direct care nursing but supports the stroke program as a stroke coordinator. Candidates must have experience as a stroke coordinator at a hospital certified as a Comprehensive Stroke Center or as a primary stroke center performing mechanical thrombectomies.
