Inpatient Registered Nurse Care Manager | Full Time

Concord Hospital Health System

Summary The Inpatient Nurse Care Manager, in collaboration with the multidisciplinary team, is responsible for assessing, planning, implementing, and evaluating patients, incorporating the clinical, psychosocial, and family factors specific to each care and disease management plan. The role involves communicating and managing transitions of care, assisting patients and families to navigate health‑care systems, recognizing and intervening to secure necessary resources, and collaborating with financial payers and insurers across the continuum of care. Education Associates in Nursing degree from an accredited nursing program; Bachelor’s degree in Nursing preferred. Certification, Registration & Licensure Current NH RN License required. Experience Acute Care Facility experience of 2 years in a clinical specialty or care management, discharge planning, quality improvement, and utilization review. Demonstrates effective interpersonal, organizational, and motivational skills, functions as a team player, and is proficient in written and verbal communication. Responsibilities Provides assessment and evaluation of patients for clinical, psychological, and social factors that may impact disease processes and determine appropriate care needs. Interfaces with patients, physicians, nurses, social workers, other hospital departments, community‑based facilities, services, and financial institutions to effect changes in case management, monitor delivery of services, and secure necessary resources. Supports patients and families, coordinates care planning and patient safety, arranges transitional care and discharge planning, and conducts utilization reviews to ensure criteria for evidence‑based clinical guidelines are met. Executes appropriate letters when medical necessity is not met and issues letters regarding patient liability. Documents measurement and evaluation of financial, clinical, functional, and satisfaction outcomes as patients progress toward goals. Evaluates and responds to the learning needs of clients, clinicians, and the community regarding discharge planning and utilization review. Manages the care and utilization review of simple to complex multi‑system patients efficiently. Provides leadership to healthcare team members regarding discharge and care planning. Performs other duties as assigned. Expectation to lift or carry less than 20 pounds, frequent moderate physical activity, speaking, bending, kneeling, reaching, sitting, standing, walking, and routine exposure to airborne contaminants, bloodborne pathogens, bodily fluids, and electrical hazards.
