Title Physical Therapist

The Physical Therapist directs the assessment, treatment planning and implementation of therapy services to patients of all ages, utilizing current knowledge, theory, evidence and sound clinical judgment. Works closely with the patient, family and other members of an interdisciplinary team to provide efficient and effective individualized patient care. Serves as educators and mentors for therapy students, peers and other medical professionals. Compensation We are committed to providing compensation information for all open positions. This position has a flat hourly rate of $57.41, based on market analysis and internal compensation factors.

Key Accountabilities

• Conducts an appropriate evaluation of the patient and documents accurate information within the medical record
• Develops and implements age-appropriate functional goals and treatment plan based on referral, the patient's history, examination findings, and patient's input
• Develops appropriate treatment interventions, considering indications and contraindications, based on patient's current level of functioning
• Provides treatment at a level consistent with current therapy practice
• Delegates and provides appropriate supervision of established plan of care to support personnel.
• Completes accurate and timely documentation of the patient's progress and the treatment performed that meets professional standards.
• Communicates/collaborates with interdisciplinary team members regarding patient progress, response to treatment, goals and program changes.
• Casual/call positions have specific requirements regarding coverage expectations.

Qualifications

• Licensed by the Minnesota State Board of Physical Therapy to practice in Minnesota
• Prefer 2 years of acute/subacute/homecare experience

Schedule

• Varying days/shifts, with flexible start and end times
• One weekend (Saturday and Sunday) per quarter is required
• Weekday availability is not required; however, additional weekday shifts may be available for those who wish to pick them up
