Job Summary
At Modani Care, we believe that everyone deserves to live independently and comfortably in their own home. As a Caregiver / Home Health Aide, you will play a vital role in providing compassionate and professional care to seniors and adults in Dawson County, NE. We are seeking dedicated and compassionate individuals to join our team and make a meaningful difference in the lives of our clients.
Responsibilities
• Provide personalized care and support to clients in their homes, assisting with daily living activities such as bathing, dressing, grooming, and medication management
• Develop and implement individualized care plans with clients, families, and healthcare teams to ensure continuity of care
• Assist with meal preparation, light housekeeping, and laundry as needed
• Monitor and report changes in client condition, medication side effects, and other relevant health information
• Maintain accurate and detailed records of care provided, client progress, and any concerns or issues that arise
• Collaborate with other healthcare professionals, including physicians, registered nurses, and occupational therapists, to ensure seamless care coordination
• Demonstrate knowledge and understanding of state-specific programs and regulations governing home care services
