When our neighbors are hurting, we want to help with the healing. ELEVATE is hiring a Community Nurse to be part of our Assertive Community Treatment (ACT) team in Southeast Iowa. This is a high-impact role for a nurse who wants their work to matter — assessing and monitoring ongoing medical, psychiatric, and social needs while coordinating care across a multidisciplinary team dedicated to elevating every life we touch.

What You'll Own

• Assess physical health care needs and coordinate with members' health care professionals
• Ensure care for those with complex mental health needs is appropriate, coordinated, and follows best practices
• Act as the team expert on medical issues for program members
• Utilize patient registries, population management, and gaps in care analysis tools
• Communicate and coordinate with primary and specialty care providers to plan member care
• Assist in coordinating medically related service needs and supports
• Implement preventive and remedial health and wellness plans
• Supervise medication monitoring and administration by staff, including training
• Complete nursing assessments and medication reconciliation for clients
• Conduct medical, health, and wellness assessments to identify health care needs
• Provide medical and psychiatric education to clients and their families
• Track required healthcare assessments and screenings
• Cross-train team members in health and wellness interventions
• Provide clients with community resources
• Follow and enforce ACT programming standards
• Travel to Henry, Des Moines, Lee, Louisa, and Washington counties as required

✅ What You Bring

Required:

• Active RN or LPN license — State of Iowa
• 2+ years of nursing practice with complex mental health populations
• Knowledge of psychotropic medications and DSM diagnoses
• Valid driver's license (travel required across Southeast Iowa counties)
