Home Care RN Case Manager

Saint Elizabeth Home Care employees work one-on-one with clients in their homes, creating impactful relationships and delivering meaningful quality care. The Home Care RN Case Manager contributes to our Mission of Caring by providing compassionate direct patient care to clients in a one-on-one setting in their homes. Day-to-day nursing activities include assessment, education, and other coordinated care. The case manager works within a cohesive interdisciplinary team with ample administrative support and uses point-of-care devices (iPad) and necessary medical equipment. Qualifications Current unencumbered RN license in Rhode Island. Minimum of one year nursing experience; home care experience preferred. CPR certification. Reliable transportation, current clean driver's license, automobile insurance.
